Tretinoin

Topical tretinoin for control of acne so long as local irritationdoes not interfere with wear of life-support equipment. The most common retinoids are tretinoin and adapalene, found in prescriptions such as avita, renova, retin-a, tazorac, and differin.

CASE 1 A 22-year-old woman presented with a 2-year history of an eruption on the anterior aspect of her neck and right arm that was relatively asymptomatic. She had a history of cystinuria, which had been treated with D-penicillamine for several years, but the D-penicillamine therapy had been discontinued 2 years before the onset of the eruption. A biopsy was performed approximately 1 year before her presentation to our institution. The biopsy specimen demonstrated clawlike downgrowths of epidermis surrounding collections of amorphous basophilic debris and hyperplastic elastic fibers. Many elastic fibers were noted to be pushing through epidermal channels Figure 1 ; , a finding that was consistent with the clinical diagnosis of elastosis perforans serpiginosa EPS ; . The patient was treated unsuccessfully with several modalities in a sequential fashion, including liquid nitrogen cryotherapy monthly for 6 months, topical tretinoin gel nightly for 2 months, oral isotretinoin at dosages ranging from 40 to 60 mg d for 15 weeks, and 2 sessions of carbon dioxide laser surgery. On physical examination, she was noted to have erythematous annular and arcuate keratotic plaques on the anterior aspect of her neck and right arm Figure 2 ; . CASE 2 A 56-year-old woman was referred to our institution for evaluation of a prutitic "nonhealing scar" that had been present on the posterolateral aspect of the left side of her neck for 1 year. The lesions developed 1 month after a revised facial rhytidectomy scar revision. The patient had been treated with oral cephalexin for 2 weeks and topical erythromycin solution for several weeks, without improvement. Her medical history revealed that she had been treated with D-penicillamine for Wilson disease for more than 20 years. The D-penicillamine therapy had been discontinued 2 months before her presentation to our clinic. Drugs have always been a mainstay in the clinical management of acne, and new drugs in the pipeline include higher-strength topical retinoids. Retinoids are chemical analogs of vitamin A and have been used in the treatment of acne for decades. In 1987, the discovery of nuclear retinoid receptors led to a new class of retinoids that include molecules to bind to and activate these receptors. Among these receptor-selective topical retinoids are such new drugs as adapalene Differin ; , tazarotene Tazorac ; and tretinoin Retin-A ; . When using topical retinoids, one issue has been to minimize cutaneous irritation without sacrificing efficacy. New vehicle systems, such as tazarotene gel and tretinoin in both gel and cream formulations ; have been developed to address this point. Dapsone DDS ; , a sulphone antibiotic agent, is actually an old drug used to treat Hansen's disease, inflammatory dermatoses and acne. It has resurfaced as a potential new therapeutic agent in topical form and excellent results have been reported in one study using topical dapsone in the treatment of acne inversa.1 Combination approaches clindamycin gel [Cleocin T] with a. To go, for men, although some physicians may disagree with me. I usually recommend 100-200 mg intramuscularly every two weeks or so. Oral testosterone preparations available in the US may be hepatotoxic, and the creams and gels tend to be pretty low-dose, although many men prefer the ease of topical application. Another approach is to use VRP's sublingual AndroSpray, usually combined with BioDIM or I3C. Ward Dean, MD.

According to CLSI recommendations, ESBL producers should be reported as resistant to fourthgeneration cephalosporins. These antibiotics may have the potential to treat mild-to-moderate infections caused by organisms with an SHV-related phenotype, whereas the drugs are not suitable for eradication of CTX-Mproducers [62]. Meanwhile, considering the increasing threat of carbapenem-resistant A. baumannii in Taiwan and limited alternative choices of drug against ESBL producers, some clinical laboratories continue to report the susceptibility to cefepime of an ESBL-producing organism and orlistat. The study validated the novel concept of developing complementary skin care products to codispense with efficacious prescription oral and topical acne therapies. This new approach to acne treatment rose out of the recognition that skin care products have a profound cutaneous effect with the ability to enhance or detract from prescription therapy. For example, topical tretinoin 0.025% cream represents one of the most effective topical therapies for prevention of microcomedones, the acneinitiating lesion. Many adolescents and young adults discontinue tretinoin within the first 2 weeks of use because of the unwanted side effects of dryness, flaking, and redness; however, 2 weeks is an insufficient duration of use to appreciate the therapeutic benefits. This study examined a codispensed cleanser and moisturizer designed to complement retinoid side effects. The mild surfactant cleanser was able to effectively remove sebum without damaging the skin barrier, as evidenced by the lack of change in corneometry and TEWl measurements during the study. The lack of barrier damage was also prevented by the glycerin-rich moisturizer. Glycerin is a humectant that attracts water to rehydrate the retinoid-induced keratinocyte desquamation. The addition of aloe vera gel, containing the anti-inflammatory methyl salicylate, and the dipeptide, which provides a protective film over the skin's surface, further prevents retinoid damage. one of the major reasons that acne therapy can be ineffective is lack of compliance. Many adolescents are sporadic in their use of tretinoin because of the side effects discussed previously, which prevent patients from completing the facial retinization phase of treatment. The use of a complementary cleanser and moisturizer, rather than patient self-selected products, may enhance compliance, thus improving the results of acne therapy. In addition to the codispensed cleanser and moisturizers, the study also provided oral minocycline hydrochloride pellet-filled capsules and a specially formulated facial wipe, serum, and mask. These latter 3 products were designed to provide a complete skin care regimen when combined with the cleanser and moisturizer. The facial wipe was developed to appeal to the busy adolescent who needs a mild, self-contained, all-in-one cleanser and moisturizer to use after school or following athletic practice. The facial wipe is a woven fiber pad containing water that removes dirt from the face. The facial wipe also contains a mixture of humectants, glycerin, sodium pyroglumatic acid, panthenol, and sodium hyaluronate that attracts water and smoothes down desquamating corneocytes. The facial wipe and its packaging can be easily disposed of after use. The serum and mask were dispensed with oral minocycline hydrochloride. The serum is a mild moisturizer containing a botanical anti-inflammatory blend of bisabolol, silymarin, and green tea. These ingredients are designed to minimize the erythema associated with both acne and retinoid dermatitis. This botanical combination is frequently used in cosmetic products designed for sensitive skin because ingredient-induced irritation can be minimized. The mask builds upon this concept by providing further assistance with retinoid dermatitis by rehydrating the skin with glycerin and algae humectants. After 2 weeks of using topical tretinoin 0.025% cream, the blinded investigator noted a statistically significant reduction in retinoid-induced inflammation P5.039 ; and dryness P5.005 ; from using the skin care system as compared to using the bar cleanser alone. Thus, the use of complementary codispensed skin care products may improve the tolerability of topical tretinoin 0.025% cream when used in combination with oral minocycline hydrochloride.
For the treatment of acne.4 The first report detailing cosmetic skin enhancement with tretinoin was published in 1983 in Argentina.5 As the use of tretinoin became more widespread, certain serendipitous side effects were observed. The cosmetic benefits of tretinoin were equal to and sometimes exceeded other cosmetic procedures and topical modalities for cosmesis and alesse.

1. What is the difference between Botox and filler agents such as Restylane and Artecoll? Botox works quite differently from the filler agents such as Restylane, Perlane and Artecoll. Botox is a muscle relaxant. It is used mainly to treat frown lines, crow's feet and horizontal forehead lines. These lines are all caused by underlying hyperactive muscles. When the muscles are no longer able to function, the lines or furrows in the overlying skin are dramatically softened. Filler agents are gels which are injected under lines, folds or wrinkles to plump them up and fill them in. They can be used in most areas of the face including in and around the lips. 2. What anti-aging skin care products can I use if I have dry, sensitive skin and I can't tolerate some of the more irritating products such as tretinoin Retin-A ; ? Many of the anti-aging skin care products such as tretinoin Retin-A ; and glycolic acid are quite irritating to those with sensitive, dry skin. There are some products, however, which do provide anti-aging benefits without causing the associated redness and irritation in most people. Gluconolactone is similar to glycolic acid Neostrata, Reversa ; but it is more moisturizing and less irritating. Like the glycolic acid products, it causes exfoliation of the superficial skin cells. This reveals the younger, smoother skin below which gives the skin a luminosity and revives it's vibrancy. Also, by removing the dead surface skin cells, all other products that are used are better able to penetrate the skin and are, therefore, more effective. Gluconolactone should be applied in the morning. Kinetin, the active ingredient in Kinerase, is a natural substance which slows down the appearance of aging in plant cells. It has been shown to work similarly to Retin-A to improve the skin's texture and appearance while providing excellent moisturization. It comes as a cream for drier skin ; and a lotion for oilier skin ; . It can be used twice per day. Age spots, or "liver spots" as they're often called, have nothing to do with the liver. Rather, these flat, brown spots are caused by years of sun exposure. They are bigger than freckles and appear in fair-skinned people on sun-exposed areas such as the face, hands, arms, back, and feet. The medical name for them is solar lentigo. They may be accompanied by wrinkling, dryness, thinning of the skin, and rough spots. A number of treatments are available, including skin-lightening, or "fade" creams; cryotherapy freezing and laser therapy. Rretinoin cream is approved for reducing the appearance of darkened spots. A sunscreen or sun block should be used to prevent further damage. 219. Galvin SA, Gilbert R, Baker M, et al. Comparative tolerance of adapalene 0.1% gel and six different tretinoin formulations. Rate of untreated K562 cells remained at or near baseline levels for the 5-h experimental period. Minimal changes in extracellular acidification rate were seen in K562 cells treated with 150 camptothecin, although K562 cells exposed to camptothecin at 300 showed a decrease in extracellular acidification rate, albeit this did not fall below 80% of baseline in any experiment. To determine if decreased metabolic rate reflected by reductions in extracellular acidification rate was a consistent, proportionate cellular response to pro-apoptotic stimuli, we examined whether taxol would induce similar alterations as those observed with camptothecin. U937 cells showed a 20% to 30% decrease in extracellular acidification rate over 5 h after a 60-min exposure to taxol 10 100 ; , but unlike the change in extracellular acidification rate seen after camptothecin treatment, that observed after treatment with taxol was not biphasic Fig. 3A ; . K562 cells treated with taxol for 60 min did not show significant alterations in extracellular acidification rate during the first 5 h Fig. 3B ; or during the complete 8-h experimental period not shown and danazol. Women, are significantly less likely to use hormonal birth control and more likely to experience menstrual dysfunction, including irregular periods, amenorrhea, and long menstrual cycles. In addition, we found that women with type 1 diabetes experienced later menarche 10; 38 ; . A limitation of this study is that the use of hormonal birth control was assessed in a single question regarding use of oral contraceptives, Depo-provera and Norplant. As described in the methods, the use of Norplant and Depo-provera by themselves are rare in this cohort. As a result, while there may be some misclassification, the main hormonal birth control method used by women in this cohort is oral contraceptives. The effect seen in this study would be biased in the direction of the null if some users of progestin are misclassified as oral contraceptive users. In conclusion, the results of this study suggest that hormonal birth control therapy may be associated with a reduction in the development of atherosclerosis in all women, and the strength of this association may be greater in women with type 1 diabetes. This finding is important since women with type 1 diabetes are at increased risk for CAD, and may also be more likely to have estrogen deficiency, menstrual dysfunction, elevated androgens, and early menopause. In addition, our data also suggest that hormonal birth control use in premenopausal women may protect against the development of fatty streaks, thus reducing the development of atherosclerosis and calcified plaque in the coronary arteries later in life.

Counsel for the College submitted that an appropriate penalty would include a four-month suspension, an oral reprimand and conditions on the member's Certificate of Registration. The member's counsel emphasized that the outcome to the client was a deterrent to the member and that the length of the penalty suspension would not affect the possibility of future error. Counsel indicated that the incident was caused by both systemic and personal error and that the member had no previous workrelated incidents. He noted that the member had already served a threemonth suspension without pay and asked the Panel to focus on the member's rehabilitation to ensure that she would return to practice and contribute to the nursing profession. Ticosteroids to control asthma symptoms. This randomized, double-blind trial included 85 patients with moderate to severe asthma who took at least 1, 500 g d of inhaled beclomethasone dipropionate. They received with suplatast tosilate, 100 mg 3 times daily, or placebo for 8 weeks. Adding suplatast tosilate to inhaled corticosteroid increased FEV1 to a significantly greater extent than adding placebo. Asthma symptoms, variation in peak expiratory flow, and serum eosinophil cationic protein and IgE also showed greater improvement with active treatment. During a steroidreduction phase, the placebo group showed a progressive reduction in peak expiratory flow, compared with no change in the suplatast tosilate group. There were few side effects. Suplatast tosilate can improve pulmonary function and symptom control in patients with moderate to severe, steroid-dependent asthma. As a result of these benefits, the inhaled corticosteroid dosage can be reduced without compromising disease control. The steroid-sparing effects of this Th2 cytokine inhibitor must be studied in other groups of asthma patients. COMMENT: This Japanese double-blind, randomized, parallel-group study evaluated 77 patients with moderate to severe asthma taking high doses of inhaled beclomethasone 1, 500 g d or more ; . Patients were treated with suplatast tosilate ST ; , a selective Th2 cytokine inhibitor, at doses of 100 mg 3 times daily or placebo. Patients treated with ST showed improved pulmonary function, symptoms, diurnal variation in peak expiratory flow, serum eosinophil cationic protein, and total IgE. There were minimal adverse effects. Since this study focused on only a small subset of asthmatics, its results are not generalizable. Future studies with higher doses of ST may be more compelling. E. J. B. For boomers, who are counting on living longer than any generation before them, health complications from being overweight could reverse that longevity. "From our analysis of the effect of obesity on longevity, we conclude that the steady rise in life expectancy during the past two centuries may soon come to an end, " stated a 2005 report by the New England Journal of Medicine on the possible decline of life expectancy in the 21st century. Each year Trust for America's Health TFAH ; , an advocacy group that promotes increased funding for public health programs, examines state obesity rates and government policies. Though the federal government instituted a plan to trim the national waistline 15 percent by 2010, 31 states show an increase in their obesity rate since last year, according to the TFAH report, "F as in Fat: How Obesity Policies Are Failing in America, 2006." Colorado, the leanest state with just 16.9 percent of its adults considered obese, is up slightly from last year. Even Massachusetts, the second-leanest state with an 18.6 percent obesity rate, is a tad more than last year. The only state that experienced a decrease in the percentage of obese adults from last year was Nevada. "This epidemic is a nationwide epidemic. Some states are higher, some populations have it higher . the trend has been up for every ethnic group, the trend has been up for rich and poor, " Dr. Jeffrey Koplan, a former Centers for Disease Control and Prevention director, told the Associated Press. Studies of the controlled release of drugs for their extended and safe use have recently become an important field of research 1 ; . Among controlled-release devices, osmotically driven systems hold a prominent place because of their reliability and ability to deliver the contents at predetermined zero-order rates for prolonged periods 27 ; . Osmotic pumps OP ; are standard dosage forms for a constant-rate drug delivery 810 ; . Preparation of an elementary osmotic pump consists of the core containing the active material and a semipermeable membrane that coats the core, having a microdrill produced orifice in order to release the active material. When the system happens to be inside the gastrointestinal tract, the fluid enters the core through the membrane and dis.
THE RAND CORPORATION QUALITY ASSURANCE PROCESS Peer review is an integral part of all RAND research projects. Prior to publication, this document, as with all documents in the RAND technical report series, was subject to a quality assurance process to ensure that the research meets several standards, including the following: The problem is well formulated; the research approach is well designed and well executed; the data and assumptions are sound; the findings are useful and advance knowledge; the implications and recommendations follow logically from the findings and are explained thoroughly; the documentation is accurate, understandable, cogent, and temperate in tone; the research demonstrates understanding of related previous studies; and the research is relevant, objective, independent, and balanced. Peer review is conducted by research professionals who were not members of the project team. RAND routinely reviews and refines its quality assurance process and also conducts periodic external and internal reviews of the quality of its body of work. Daniel A Carrasco, M.D.; Suhail M Hadi, M.D.; James Spencer, M.D.; Mark Lebwohl, M.D.; Mount Sinai School of Medicine, New York, NY Objective: To study the effectiveness of the new excimer laser for the treatment of vitiligo. Patients: Thirty-two patients with 55 spots of vitiligo adults and children, males and females, with different ethnic backgrounds were studied. Populationbased sample was studied. Setting: The procedure was performed at the Department of Dermatology at the Mount Sinai Medical Center in New York City, which is an institutional medical center. Design: The treatment was started with the lowest dose, which is 100 mJ cm2 comparable to one minimal erythema dose MED ; value and one multiplier ; . Depending on Fitzpatrick skin type, the dose was raised gradually in a step-wise fashion. In skin types I-II, the same does was repeated twice before going up to avoid burns. Patients were treated for 30 sessions, or 75% repigmentation, whichever comes first. Results: Overall 55 spots were treated, 29 52.8% ; had 75% pigmentation or greater, 35 63, 7% ; had 50% pigmentation or greater. The best results were on the face; of the 21 spots treated 15 71.5% ; had 75% pigmentation; 16 76.2% ; had 50% pigmentation or greater. Other areas neck, extremities, trunk, genitals ; had moderate response in comparison to the face. The least response was on the hands and feet, of the 5 spots treated only 20% showed 50% pigmentation or more. Conclusion: Slightly more than 50% of the patients tested showed 75% or more pigmentation of their lesions, after 30 treatments or less, which makes the excimer laser an effective method of treatment for vitiligo. P568 Long-Term 12-month ; Safety and Efficacy of Triple Combination Agent 0.01% Fluocinolone AcEtonide + 4% hydroquinone + 0.05% Tretinoni ; in the Treatment of Patients with Melasma of the Face. Actinic Keratoses eliminated 75-80 percent of AKs initially present. In one study, 74 the results of using 1 percent 5-FU and 5 percent 5-FU were similar. In some trials, 69, 74 a longer course of treatment was needed to treat AKs on the hand where 5-FU is less efficacious ; than on the face or scalp. Two of these trials reported results at followup periods longer than 6 months.71-73 The longest followup evaluation of 5-FU treatment was published in 1972; 74 it reported that 25 percent of patients had recurrent or new AKs within 2 years of treatment, and 50 percent within 3 years. In a trial of 15 patients performed in the 1990s, eight were available for evaluation 32 months after treatment with 5-FU on one side of the face and TCA peel on the other side.71, 73 At 32 months, 5-FU reduced the mean number of AKs from 18 before treatment to 10. In one case series patients were treated with 5-FU 1-2 days a week for an average of 6.7 weeks. The first six of 11 patients reaching the 9-month post-treatment marker retained an 86 percent clearance of actinic keratoses.53 In the second series, patients were treated with 1 percent 5-FU; at 2-year followup, 25 percent of one cohort and 40 percent of another required re-treatment with 5-FU for recurrence of AKs.74 A sixth report75 examines a newer formulation of once daily 0.5% 5-FU cream. Two phase-3 trials described in an FDA document reported short-term results. One month after a 4week course of treatment, 64% and 78% of patients had at least 75% fewer lesions on the face and anterior scalp. Three controlled trials examined the efficacy of tretinoin in the treatment of AKs.70, 76, 77 One study compared the efficacy of tretinoin and 5-FU vs. 5-FU alone on the upper extremity. The side treated with 5-FU plus tretinoin reduced the mean number of AKs per patient from 15.7 to 3.4.70 In the other two studies, tretinoin reduced the number of AKs more effectively than placebo.76, 77.
